Dear teachers,
I want to write a thank you letter to the bus company for their help to my lost student card.
The company's manager is named Hisham B, but I don't know his/her gender and surname.
I don't know how to write the appropriate salutation at the beginning of letter.
Here are what I can think of:
Dear Hisham B
Dear Mrs B / Mr B (I guessed his family name starts with B but his company doesn not want to show it)
Dear Sir / Madam
Dear Hisham
Please help me, teachers.
Thank you.

Tricky because I'm not even sure if Hisham is first or last name. If you can't phone up and ask the switchboard, you could use:
Dear Sir or Madam
