I imagine (a) is the intended answer, but it is rather context-dependent.

6 a) he didn't answer; b) he doesn't answer; c) he hasn't answered; d) doesn't he answer; e) all the above variants are wrong.

Grammatically, (a), (b) or (c) could fit, but I imagine (b) is the intended answer (assuming that 'he' is the addressee).
