Hi :) I am doing a questionnaire in English but I am not sure with some questions. Can you tell me if it is written correctly or how should it be? Thanks :)

1. What do you think of when you say “Charity”?
2. Do you know forms of their funding?
- Yes, I am interested in it

3. Have you ever met personally with some form of collecting money?
- Yes, I contributed

5. Do you know what is collected money really used for?
- Yes, it is easy to find
- No, I do not know how to find these information
- No, these information are very difficult to find

6. What organizations do you contribute to?
- Large organizations (UNICEF, the Red Cross,...)

7. For what purposes do you contribute?

Not sure what you meant by this.

"Do you know what form their funding takes?"
