Bent over

Dear teacher,

But for people who are worried about ending their days bent over with the disease, or with a broken hip in a nursing home, Dr. Carmona has a message.

Q : What is modified by "bent over"? I think it specifies "ending", showing a situation they may have in the end of their life.

What do you think?

'Bent over' is a predicative adjective phrase referring to 'people'.
