Either might be possible, depending on context and intention. 'Also' is a very mobile adverb, but, like 'only', tends to go next, or as near as possible, to the word on which the speaker is focusing.
Thus
It was also MY decision.
suggests that other people made the same decision,
whereas
It was my DECISION also.
could imply that it was e.g. my wish, as well as my decision.
