Yes, this idiom is very much in use today. You can say that a person or a thing that gives you trouble is a pain in the neck, eg:

- Having to convert my accounts from dollars to euro is a real pain in the neck.
- It's a pain in the neck having to delete all the spam in my email.
- John's being a right pain in the neck. Can't you tell him to go away.

Your example is good. You could also say something like:
- Answering a phone call at midnight is a right pain in the neck.
- It's a real pain in the neck having to answer a phone call at midnight.
