I'm going to apply for a part time job in an English center. I have to write an application in english but I'm not sure about what I should write.
I intend to introduce myself (my name, where I come from..) and mention the reasons why I choose that jobs. Should I add something else?
And I don't know how to write a CV. Should I list the things I did in seperate lines or should I write a paragraph?
Please help me.

Your CV should be laid out properly if it is extensive; if you do not have much work experience, then you can include it as a paragraph in your application (cover) letter. Post it here, too, for review.
