give, present or your suggestion

I am going to visit my former school teacher who has been recently promoted to be the vice principal.
Here is what I have written:

"Please arrange a time at your convenience so that I can [give / present / your suggestion] you a survey form to fill in"

I want to replace the word [give / present] that can show my respect to his vice principal position.
Please help me.

There is nothing wrong with "give" but you can make the request more respectful by adding something like: "....if that's not too much to ask."
