In my humble opinion "who" is a singular pronoun here so I would use "takes". So,

1) Who takes the children to the nursery in your family? (Do we need a coma after "nursery" in this sentence?)

In addition, in the passive voice:

2) Who the children are taken by to the nursery in your family? ( Do we need a coma after "nursery" in this sentence?)

3) Whom the children are taken by to the nursery in your family?

4)By whom the children are taken to the nursery in your family?

5) In your family, who the children are taken by to the nursery?

6) In your family, whom the children are taken by to the nursery?

7) In your family, by whom the children are taken to the nursery?

Are all of these senteces grammatically OK and which percent of NES would use 1), which 2) and so on?

What you find confusing has something to do with the word agreement.
While the word, "who" is both singular and plural, it is treated as singular.
Rule: if the noun or the subject is in the singular form, the verb has to be in the singular form. In the same way, if the noun is in the plural the verb must also take the plural form of the verb.

In your given example above, the interrogative pronoun "who" which is singular in form used the verb "takes" which is in the singular form of the verb take.

Following the rule in word agreement the verb "take" is the plural form of the verb take therefore, cannot be used to replace the verb "takes" on your example above. .

The word "takes" will only use when the subject is singular. For example; All it takes is practice. While the word "take" will only use when the subject is plural. For example; My friend and I take English every semester.
