Capitalizing Unknown Pronouns

Why is the word "doctor" not capitalized in this sentence: "They showed him to their doctor, who said there was little chance Charles would ever reach normal height." We know the doctor is a specific person, even though we don't know his or her name.

It is a common noun here.

If you are naming the doctor, or addressing him/her (with or without the name), then you capitalize.
