Grammar of a Formal Sentence

Dear teachers,

The following is a very formal sentence. May I know whether it is grammatical.

1. I shall forward you proofs of my expenses that I, as recently as on the 27th of Dec, have personally incurred, and have helped to defray the medical costs of someone.

Not entirely idiomatic, but basically fine and comprehensible.
