Adjective at the beginning of sentence

"Fearful that the new rulers would carry out such abuses, thousands of Afghans have raced to Kabul’s airport and border crossings to flee the country."

Why is this sentence started by an adjective but not a noun/adjective clause ?
What structure is used in this sentence ?

It means 'Being fearful...', with ellipsis of the participle.
