Is (a) grammatically correct?
The idea is that one person who is a doctor and a philanthropist is waiting in the lobby, but it seems to me that (a) doesn't work.
'A doctor and a philanthropist' means two people not not.

Not correct.
To designate the same person, we would say 'A doctor and philanthropist...'.
To designate the same person, we would say 'A doctor and philanthropist...'.
