You can say this if someone's done something really stupid or silly.

For example

  • "I broke Mum's vase."
    "That was her favourite vase! Now you've really gone and done it, haven't you?"

  • "Jacob's been arrested for selling drugs!"
    "For selling drugs? Jeez, he's really gone and done it this time."

Note:
Mostly used in the present perfect tense, as in "has really gone and done it" or "have really gone and done it".
