1. a) Who did you stay with?
b) With who did you stay?

2. a) How long do you want a visa for?
b) For how long do you want a visa?

3. a) What date are you arriving on?
b) On what date are you arriving?

4. a) What time do you usually come home at?
b) At what time do you usually come home?

5. a) Who are you waiting for?
What are you waiting for?
b) For who are you waiting?
For what are you waiting?

6. a) What is it made of?
b) Of what is it made?

Do you honestly want to know how to construct questions? The above examples above will not help you at all. Get yourself a good grammar book such as Roundup 5 or a Murpy's and come back if you are still confused.
The English language can be sophisticated, but for second language speakers keep to the basic guidelines of the grammar book. Begin your sentence with, who, which, where, when, what, and how. Practise changing positive statements into questions.

Who are you staying with?
I come from Scotland
Where do you come from?
English is one of the easiest languages in the world, so no need to make it complicated.
