past simple for long lasting actions in the past

Is it possible to say "She looked after him all through his coma. Or it's a must to use past perfect continuous :She had been looking after him all through his coma. Thank you in advance

We can say " He worked for 5 years as a teacher" He worked through all 5 years as a teacher. Is it the same or not?
