Someone is "out of his element" when he has to do something that he is not familiar with and he feels very unpleasant and uncomfortable. Am I right?

This phrase simply means to be doing something that one isn't familiar with or doesn't enjoy doing. A ship's Captain would not be familiar with navigating an aeroplane ergo he is "out of his element" even though in theory the principles of navigation are the same.
In common use however the phrase isn't used in such extreme circumstances and is more likely to be used in this way "John hates washing cars but has taken a job in a car wash he is absolutely out of his element"
Another way of expressing this is to say "he or she is like a fish out of water"
